Information sharing

How the NHS and care services use your information

Hassengate Medical Centre is one of many organisations working in the health and care system to improve care for patients and the public. 

Whenever you use a health or care service, such as attending Accident & Emergency or using Community Care services, important information about you is collected in a patient record for that service. Collecting this information helps to ensure you get the best possible care and treatment.

The information collected about you when you use these services can also be used and provided to other organisations for purposes beyond your individual care, for instance to help with:

  • Improving the quality and standards of care provided
  • Research into the development of new treatments
  • Preventing illness and diseases
  • Monitoring safety
  • Planning services

This may only take place when there is a clear legal basis to use this information. All these uses help to provide better health and care for you, your family and future generations. Confidential patient information about your health and care is only used like this where allowed by law.

Most of the time, anonymised data is used for research and planning so that you cannot be identified in which case your confidential patient information isn’t needed.

You have a choice about whether you want your confidential patient information to be used in this way. If you are happy with this use of information you do not need to do anything. If you do choose to opt-out your confidential patient information will still be used to support your individual care.

Data being used or shared for purposes beyond individual care does not include your data being shared with insurance companies or used for marketing purposes and data would only be used in this way with your specific agreement.

Health and care organisations have until 2020 to put systems and processes in place so they can apply your national data opt-out choice. Our organisation is not currently’ (2018.10) able to apply your national data opt-out choice to any confidential patient information we may use or share with other organisations for purposes beyond your individual care.

This practice may supply personal health data to comply with its legal obligations from time to time, as directed by the Secretary of State for Health, or other recognised Statutory Authority.

Anonymised data on the use of fit notes is being provided to the HSCIC on behalf of Department of Health, and the Department for Work and Pensions. This will enable the Department for Work and Pensions to undertake research analysis to inform policy relating to employment and sickness absence, including evaluation of Fit for Work.

Facilities & Access

Our premises are purpose built with wheelchair access throughout, toilet facilities suitable for people with physical impairments and baby changing facilities.

Outside there is space for buggies and car parking spaces, including designated spaces for those with disabilities. 

If you suffer from any form of impairment or have any special needs and may need help in using the service we provide, please make yourself and your needs known to our reception staff - we will do all that can reasonably be done to help. If you are likely to need help or assistance when visiting us, please bring a friend. See our “Guide for our patients with disabilities”.

To Register for On-line appointments / repeat medication as well as your summary information

Please contact the practice to obtain your own pin and other details, to enable you to log in to book appointments, order repeat medication or let us know you have changed your address or telephone number.

You can also have this access for your children, automatically upon application up until they are 11yrs old. Once they are approaching 11yrs if parent access is to continue both the parent and the "child" will nned to speak to the practice manager - so that due diligence is undertaken and the "childs voice" is obtained.

Should you want full access to your records created from 1st April 2015

Please contact the practice receptionists and read and complete the specific form that will allow you to obtain your own details. This facility will not, usually, be available to patients under the age of 18yrs.

Safeguarding Statement 2024

Hassengate Medical Centre is committed to protecting Children and Vulnerable Adults and we will make their welfare our highest priority. Where additional support is necessary this will include working with other agencies. If you have any concerns about a child please speak to Dr Ahmed or Vulnerable adult please speak to Dr Hodge. If neither are available please speak to any GP here as all are involved in supporting in Safe Guarding situations

PUBLICATION OF EARNINGS 2022/23

“All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.

The average pay for GPs working in Hassengate Medical Centre in the last financial year was £58,478 before tax and National Insurance. This is for 2 full time GPs and 8 part time GPs who worked in the practice for more than six months.

However, it should be noted that the prescribed method for calculating earnings is potentially misleading because it takes no account of how much time doctors spend working in the practice and should not be used to form any judgement about GP earnings, nor to make any comparison with any other practice.”
